Fees & Insurance

All necessary payments are expected in full at check-out. This includes:

  • Co-pays for insurance companies with which we have a contract.
  • Full payments if we do not have a contract with your insurance company or if you have no insurance. However, if you have no insurance and you pay in full on the date of your visit, you will receive a 15% discount on that bill.
  • Payments for outstanding balances for telephone, fax, or email/Patient Portal consultations or treatment.
  • You may pay your balance with cash, check, or credit card (Visa, Mastercard, debit card).
  • Delinquent payment plans must be paid before care is provided.
  • Outstanding balances sent to collections must be paid before care is provided.
  • If you choose to pay by check and your check is dishonored, you agree to pay a service fee of $25.00, or any higher amount allowed by law, and we may electronically debit or draft your account for this fee.
  • If you are unable to pay in full, please request an arrangement with us before the day of your appointment; otherwise, we may reschedule your appointment or notify your insurance company.
