Medical Exams & Procedures

Please share with us any medical concerns you have. We encourage you to ask questions and work collaboratively with us to provide you the best care. You can expect evidence-based care from your clinician and care team, as well as support for self-managing your health and healthcare. Be as clear as you can early in the visit about what you would like to address. If you have forms needing completion, then please have your portion completed first.

What to bring with you: To more effectively serve you as a patient, we ask that you bring a list of all medications that you are taking, including prescription drugs, over-the-counter drugs, herbal medicines, supplements, and any other kind of conventional or alternative medicine. Also, we ask you provide any information about visits to specialists, data from recent hospitalizations, recent lab results, and/or any self-care information received outside of our practice.

Lab Work: Lab work is done in our office without an appointment before 11:00 am, on a first-come-first-serve basis. Patients with scheduled appointments will have lab work done after their appointment.

Video, Portal and Phone Consultations:

  • Though the best way to evaluate and determine treatment for most conditions is with an appointment in the office, some conditions can be appropriately managed with video visits or communication through the patient portal. If you have been seen in the office in the past 12 months, then we can arrange visits using video or phone. There is a fee for these services.
  • The best way to contact staff and providers is via the patient portal. We encourage all patients to register and use the portal. Information shared in the portal is encrypted to maintain your privacy (HIPPA compliant). Regular email is not a secure and legal way for us to communicate.
  • Messages left for staff by phone are responded to as quickly as possible. Please request of routine medicines refills through your pharmacy. They will send us an electronic request for approval. Please allow 48 hours for medication refill requests.
  • We will also be offering Patient Portal consultations in the near future!
